Prosthodontic Consultation

Some patients may have received a referral from their general dentist or specialist doctor due to a unique problem or a complex circumstance. Others may be seeking specialist care or a second opinion on their own. Regardless of your avenue of introduction, your initial appointment will consist of a consultation with Dr. Michael Kopecky, our prosthodontic specialist, exploring your treatment needs and understanding your expectations. For those who are interested, family members are welcomed by the doctor at this and future appointments.

Please assist us by providing the following information at the time of your consultation:

1. Your referral slip and any current x-rays. If x-rays are not available, they may need to be taken at this appointment.
2. Completed office forms.
3. Medical or dental insurance cards.
   * Regardless of insurance benefits, fees for all first appointments are required at the time of the visit. Claims are submitted and reimbursed upon receipt.
