Privacy Policy

HIPPA forms outline our strict observance of privacy policies

• These forms are included with the initial patient packet.
• Signed forms are needed for patients of all ages. For minors, parents/guardians should print the child’s name in the patient line and sign their own name in the signature line.
• When a child of record reaches 18 years old, new forms are signed by the now adult patient.
• Permission may be granted by individuals for representatives from Cedar Lake Dental Professionals, Ltd. to discuss treatment with parents, spouses or other specific individuals by listing their individual names.
